Team Leader (ACT)

We are currently searching for a qualified Team Leader. (ACT) The Team Leader establishes, administers, and directs ACT program, a self- contained team which assumes responsibilities for directly providing needed treatment, rehabilitation, and support services to identified consumers. The Team leader supervises and evaluates the multidisciplinary team, in conjunction with appropriate psychiatric support, and functions as a practicing clinician on the team.

Responsibilities & Duties:

• Direct the day to day operations of the ACT team including scheduling staff to ensure appropriate coverage for day, evening, weekend, holiday shifts, and on-call hours.
• Direct and coordinate treatment, rehabilitation, and support services in collaboration with the psychiatrist.
• Evaluate consumer status and conduct appropriate planning and coordination of treatment activities to ensure immediate attention to consumer strengths.
• Coordinate and supervise the consumer centered comprehensive assessment process.
• Conduct clinical assessments and provide direct clinical treatment, rehabilitation, and support services to consumers as needed.
• Coordinate and supervise admission, assessment, and treatment plan development.
• Facilitate and monitor discharge planning in collaboration with consumer, family, psychiatrist, team members, and relevant outside requirements.
• Review assessments and treatment plans for appropriate clinical content.
• Establish effective relationship with consumers, families, treatment team members, and designated government and managed care officials.
• Assist in development and implementation of program budget.
• Perform on- call duty for crisis intervention, staff supervision and consultation.
• Oversee management of medical records ensuring compliance with company policy, Medicaid and other third-party requirements.
• Initiate and maintain relationship with law enforcement, human service agencies, and community resource networks.
• Utilize safe crisis management techniques to manage situations involving agitated and or hostile behaviors from consumers, without the use of physical restraints.
• Aid with training related to medical records requirements and monitor staff compliance with medical records procedures.
• Participate in staff recruitment and selection process.
• Develop and conduct staff orientation and training.
• Ensure facilities are operated in a clean and safe manner.
• Maintain consumer confidentiality per company policy, HIPAA Privacy Rule regulations, and the Department of Public Welfare.

Qualification & Skills:

• Master’s degree in any of the following fields: Nursing, social work, rehabilitation counseling, psychiatric rehabilitation, or psychology.

• PA licensed Social Worker (LSW/LCSW), Marriage and family Therapist, Professional Counselor (LPC) preferred but not required

• Must be a Certified Peer Specialist Supervisor, or willing/able to obtain such certification.

• Must have at least 12 hours of specialized training in providing integrated services to persons with co- occurring mental illness and substance use disorders

• Two years’ experience providing treatment to persons with sever and persistent mental illness.

• Minimum of two years’ experience in a management/supervisory role.

• Ability to be physically mobile

• Must possess the ability to perform crisis intervention techniques, including verbal and physical redirection and guidance of potentially physically aggressive consumers during crisis situations.

• Must be able to physically assist with evacuating consumers during emergency situations.

Recovery Support Practitioner

We are currently searching for a qualified Recovery Support Practitioner to join our dynamic team of experienced professionals. As a Mental Health Worker/ Recovery Support Practitioner you will assist individuals receiving services with daily living skills, social activities, community integration, self-determination, and the transition to independence.

Responsibilities & Duties:

• Provide counseling and guidance to individuals to effectively deal with problems and develop positive relationships.
• Provide support and guidance to individuals to engage in social and leisure activities and to develop/strengthen problem solving skills. Accompany and monitor individuals on community trips and appointments.
• Aid individuals with daily activities, personal care activities, cooking, and other household responsibilities.
• Document behaviors of individuals receiving services in daily report log and weekly progress note.
• Perform vital checks and report medical issues to nursing staff.
• Ensure a clean, safe and homelike environment with the assistance of housekeeping personnel.
• Encourage individuals to participate in social and leisure activities to develop and strengthen problem solving skills.
• Ensuring the health safety and welfare of the individuals

Qualification & Skills:

• High School Diploma or equivalent
• Six months of working experience in the Mental Health Field
• Valid Driver’s License
• Ability to perform crisis intervention techniques, including verbal and physical redirection and guidance of potentially physically aggressive individuals during crisis situations.

Mental Health Worker

The Mental Health Worker is a member of the Treatment Team and is responsible for carrying out rehabilitation and support functions. The Mental Health Worker assist in treatment through implementing substance abuse services, providing education, support, and consultation to families, and providing crisis intervention. This position is required to work hours that coincide with consumer needs. Evening, weekend, and holidays.

Responsibilities & Duties:

• Provide services coordinating for assigned consumers, including coordination and monitoring activities of the individual treatment team
• Work collaboratively with members of the treatment team and other service providers.
• Establish, implement, and maintain treatment goals and objectives through a devised treatment plan for each family in assigned case load.
• Document treatment data/results within daily progress notes and monthly treatment plan reviews
• Provide individual supportive therapy, social skill development, and assertiveness training to increase consumer social and interpersonal activities in community settings.
• Provide 24/7 crisis intervention support though an on-call rotation
• Transport clients using intervention support through an on- call rotation
• Maintain consumer confidentiality per company policy, HIPAA Privacy Rule regulations, and the Department of Public Welfare.

Qualification & Skills:

• Bachelor’s degree in any of the following fields: Nursing, social work, rehabilitation counseling, psychiatric rehabilitation, or psychology.
• 1-year experience working with children and families
• Valid Driver’s License
• Ability to perform crisis intervention technique

Certified Registered Nurse Practitioner

We are currently searching for a qualified Registered Nurse to join our team. The Nurse is responsible for coordination of treatment and oversight of medication management for consumers.

Responsibilities & Duties:
• Review and ensure medication management compliance in clinical and residential programs as well as oversee and make recommendation for change in service plan for consumers regarding health care needs.
• Interface with hospitals and ancillary service providers related to client care, coordination of services, continuity of care and other related matters.
• Provide first aid care to consumers as necessary and interface with pharmacies regarding the effective and efficient distribution of medication.
• Provide nursing consultation to staff regarding consumer health care matters and access the abilities of staff to appropriately perform medication management responsibilities.
• Conduct staff training and consumer groups to educate on medication and health care related topics and participate in policy development and review for policies related to medication, infection control, and health care of consumers.
• Develop and distribute advisories to staff and consumers regarding special precautions (i.e. infectious disease, flu, etc.)
• Respond appropriately to emergency situations as indicated in company policy.
• Maintain consumer confidentiality per company policy, HIPAA Privacy Rule regulations and the Department of Public Welfare.

Qualification & Skills:
• Certified Registered Nurse Practitioner
• Five years’ experience in the human services field working with individuals with developmental disabilities and or mental health disorders.
• Knowledge of medical procedures, terminology, and equipment.
• Must have strong written and verbal communication skills and a strong assessment skill.
• Must display professional attitude at all time.

Certified Peer Specialist

• Assist peers in identifying and prioritizing needs, as well as articulating personal goals and objectives for recovery as part of their individual service plans.
• Assist peers in accessing needed services; Inform peers about community and natural supports available to them.
• Assist peers in securing employment, housing as well as helping them understand benefits with a new job
• Help peers build social skills
• Support peers in overcoming job-related anxiety.

Qualification:
• Valid Driver’s license
• 2+ Year Experience working in mental health field
• Certified Peer Specialist
• Be willing to continue your education and receive certifications through the Mental Health Association of Southeastern PA.
