Children’s Blended Case Management

Children’s Blended Case Management services help children and adolescents live successfully within their family unit and the community. Individualized services are based on the unique needs of each child and family, including basic human needs (i.e. physical, emotional, spiritual, vocational and social needs).

A child’s assigned case manager provides services at home, school, medical appointments or other sites where the child needs support. The case manager coordinates all the child and family’s services and consults with mental health and educational professionals on crisis situations and overall treatment.

Case managers meet with the family once a month, at a minimum and crisis services are available 24 hours a day, 7 days a week.

To support the physical, emotional, spiritual, social and vocational needs of the child and family, CBMC services include:

  • Developing and implementing strength-based assessments
  • Developing and maintaining relationships with community based support services
  • Participating and coordinating treatment team services
  • Providing crisis support

CBMC services are available for children who:

  • Have been diagnosed with a mental illness or emotional disturbance
  • Are 3-21 years of age – young adults ages 18-21 are only eligible for CBCM if currently enrolled in a secondary level education system
  • Enrolled in medical assistance

Children must also meet one of the following:

  • Being age six or younger and enrolled in or require early intervention services
  • Receiving services, with their families, from three or more publicly funded programs (e.g. medical assistance programs, education, child welfare or juvenile justice)
  • Returning from state mental hospitals, community inpatient units or other out-of-home placements, including foster care or juvenile court placements
  • Being recommended as needing mental health services by a local inter-agency team
  • Being recommended for services by local providers and would result in an out-of-home placement (e.g. community in-patient foster home, juvenile court placements) without blended case management or are returning from an out-of-home placement
  • Having had six or more days of psychiatric inpatient treatment within the past 12 months
