Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is a comprehensive community-based treatment program for persons with severe and persistent mental illness.  Services are provided by a team of multi-disciplinary mental health professionals who provide treatment, rehabilitation and support services to individuals within the community.

The team is available to provide services to individuals 24 hours a day, seven days a week, 365 days a year.

Behavioral Health Rehabilitation Services

Behavioral Health Rehabilitation Services (BHRS) provides intensive, short-term services aimed at maintaining children in their current home, school and community environments, by helping them to learn new skills, promoting positive behaviors and improving independence and socialization.

Blended Case Management (BCM)

Blended Case Management (BCM) offers a unique and effective approach to help individuals with mental illness live successfully in the community. It links individuals with behavioral, psychological or developmental challenges with necessary community services, advocates for their needs and provides them with on-site crisis intervention and skill development. Blended Case Management care is customized to meet the needs of the individual.

Certified Peer Support (CPS)

This person-centered recovery program focuses on advocacy, education, linking, building relationships and creating a natural support system. It is provided to those who use or have used behavioral health system services.

Certified peer specialists model coping skills, foster resilience and assist in identifying strengths and areas in need of improvement. Goals are identified and a supportive plan is developed.

Children's Blended Case Management (BCM)

Children’s Blended Case Management helps children and adolescents live successfully within their family unity and the community. Individualized services are based on the unique needs of each child and family, including basic human needs (i.e. physical, emotional, spiritual, vocational and social needs).

A child’s assigned case manager provides services at home, school, medical appointments or other sites where the child needs support. The case manager coordinates all the child and family’s services and consults with mental health and educational professionals on crisis situations and overall treatment.

Case managers meet with the family once a month, at a minimum, and crisis services are available 24 hours a day, 7 days a week.

Children's Family Based Mental Health (FBMH)

Mental health treatment, family support services and case management integrated services are provided to families so they can continue caring for their child within the home. This program is for children who meet medical necessity criteria for FBHMS, require a higher level of care and are at risk of an out-of-home placement without further intervention.

The goal is to strengthen the family unit buy teaching families how to care for their child or adolescent, who is diagnosed with an emotional and/or behavioral problem, and sustain the principles taught, after treatment has ended to reduce the need for out-of-home placement.

Community Residential Rehabilitation (CRR)

Community Residential Rehabilitation (CRR) is a strength-based program that seeks to maximize a resident’s potential and quality of life. Residents are adults who experience challenges in independently living in their community. Their recovery plan needs may include, but are not limited to, mental health education, assistance with daily living activities, medication education and management, and appropriate social skills and nutrition.

Elmwood/Hemlock Drive Home Program

The Elmwood/Hemlock Drive Home Program is a six-bed, non-licensed, residential program that supports residents dealing with the challenges and difficulties of persistent mental illness.

With the help of professionally trained staff, residents live as independently as possible with training and assistance in community living skills, which serves as an integrating focus for their rehabilitation. It provides all aspects of care for its residents including assistance with activities of daily living and instrumental activities of daily living, meal preparation, medication management, medical care coordination and various enriching activities.

Long-term Structured Residence

Long-term Structured Residence provides a structured, trauma-informed recovery environment for adults dealing with the challenges of persistent and serious mental illness.

The program empowers individuals to make choices, attain life goals, build relationships and participate in community life to the best of their abilities and in the most integrated and independent manner that is safe for them. Staff work with individuals in developing a recovery plan that includes rehabilitative goals based on the unique hopes, dreams and desires of each individual.

Personal Care Homes (PCH)

Personal Care Homes (PCH) provides a home-like environment for individuals diagnosed with severe and persistent mental illness in a residential setting.

Staff provides assistance with daily living, transportation and scheduling medical appointments. Residents are provided with three well-balanced meals a day, weekly room cleaning, laundry, medication management, along with individual and group leisure activities.

PCH is licensed by the Pennsylvania Department of Human Services.

Psychiatric Rehabilitation

Psychiatric Rehabilitation services focus on recovery and wellness. Person-centered services aim to improve all aspects of leading a meaningful life, including living, learning, socializing, working and self-maintenance. Individuals learn strategies to reduce life’s interruptions and understand and create a foundation for wellness, complete personal wellness and crisis plans, and gain continued support.

We work collaboratively with individuals, their family and resources within their community, to identify opportunities for skill building and growth. Every individual is actively involved in the design, development and management of  a successful recovery plan.

School-Based Outpatient/Outpatient Services

Salisbury Behavioral Health brings outpatient services to elementary, middle and high school students at schools in the Catasauqua School District. We help students with emotional and/or behavioral issues by bringing a therapist to them. This approach improves access and removes barriers to treat children, youth and young adults.

Supported Living Program (SLP)

The Supported Living Program (SLP) provides a structured recovery environment for adults dealing with the challenges of persistent mental illness.

Structure is offered through crisis support, medication management, training
and support, and peer support as identified by the resident’s individual recovery service plan.

SLP empowers individuals to make life choices, attain life goals, build relationships and participate in community life to the best of their abilities and in the most integrated and independent manner for the individual and his/her
community.

The program provides mental health services that are resident-centered, culturally competent, individualized, flexible, coordinated, accountable and strength based, with the goal of providing the opportunity for people to recover
with the recognition that the recovery process is unique for each individual.

SLP supports individuals to live as independently as possible by providing training and assistance in community, living skills and serving as an integrating focus for an individual’s rehabilitation.
