Psychiatric Rehabilitation services focuses on recovery and wellness. Person-centered services aim to improve all aspects of leading a meaningful life, including living, learning, socializing, working and self-maintenance. Individuals learn strategies to reduce life’s interruptions and understand and create a foundation for wellness, complete personal wellness and crisis plans, and gain continued support.

We work collaboratively with individuals, their family and resources within their community, to identify opportunities for skill building and growth. Every individual is actively involved in the design, development and management of a successful recovery plan.

We believe everyone has the capacity for learning and growth.

The cornerstone of our program is individual choice and active involvement in all areas of recovery. Our personalized services focus on:

  • Delivering a skills-based program for supporting living, learning, working, social and self- maintenance roles
  • Building resilience by learning coping skills to adapt to adversity, trauma and life’s significant sources of stress
  • Increasing the awareness and understanding of mental and physical wellness and the value of peer support
  • Applying multicultural principles that are sensitive to an individual’s journey
  • Collaborating with community organizations to further wellness and peer support

Our programs use evidenced based and emerging best practices to meet individuals’ preferences and needs.

Services are offered individually and in groups at our facilities, in the community, in individual homes or another location of the individual’s choosing.

Individuals interested in joining the program must:

  • Choose to participate in the program
  • Meet the age criteria
  • Have a written recommendation by a licensed healing arts practitioner (e.g. physician, certified registered nurse practitioner, physician assistant or a licensed psychologist)
  • Have a history or presence of a serious mental illness
  • Experience functional impairments that interfere with daily living, learning, working, socializing or self-maintenance

Individuals must complete the following required forms prior to entering the program:

  • Referral form
  • Criteria of Eligibility form – completed by a licensed practitioner of the healing arts

Once the required forms are received, the Psychiatric Rehabilitation staff schedules a meeting with individuals to review all completed forms, workshops of interest, policies and expectations, and provides them with an orientation.
